Membership Application

Greeting Friends,

We at Clubhouse Atlanta would like to thank you for taking the time to consider becoming a member of our unique family. Clubhouse Atlanta follows the Clubhouse International model of psychiatric recovery where our members work side-by-side with staff on reaching their goals at their own pace. Members of Clubhouse Atlanta have access to opportunities leading to socialization, education, employment, housing and so much more. 

To be considered for membership, the following must be submitted: 

1. A Clubhouse Atlanta Perspective Member application and

2. A detailed psychosocial summary, current and completed by a supervised or licensed practitioner or

3. A detailed psychiatric assessment, current and signed off by an MD,  Nurse Practitioner or other licensed practitioner.

Once the above information is received, it will be reviewed, and a determination will be made. If more information is necessary, you will be contacted by a member of our team.
